First-Year & Transfer Student-Athlete Forms
All first-year and transfer student-athletes are required to fill out Sports Medicine paperwork on ARMS. You will receive an email from ARMS once you have activated your Assumption email account.
The following forms will be required for completion of ARMS paperwork:
Note: All first-year and transfer Assumption University student-athletes are required to fill out "First Year" Sports Medicine, Communications, and Compliance paperwork on ARMS before he/she may begin their initial season of competition at the college.

Returning Student-Athlete Forms
All returning student-athletes are required to fill out Sports Medicine paperwork on ARMS. You will receive an email from ARMS prior to the start of your athletics season. Please log into your ARMS account to complete all forms.
